Table of Contents

Every year, we cherish the moments to celebrate our journey and spend time with family, friends to share love, gratitude, and peace. Our life presents challenges that can cloud this joyful picture. Acute health issues, financial limitations, personal disappointments, frustrations, and family concerns can create a storm of emotions. Life-altering decisions may need to be made at a moment’s notice, leaving individuals—regardless of age—feeling anxious, overwhelmed, frustrated, angry, and at a loss. These challenges can even test the bonds that hold families and friendships together.

If this sounds all-too-familiar, know that you are not alone.

At Guide2Care, we specialize in helping people, including seniors, the disabled, and their families, make sense of the complex world of health and social care. From young adults to those in their twilight years, including planning for late-life care, we are here to provide support every step of the way. We lead with curiosity and empathy, striving to understand your unique needs and objectives. Our goal is to identify the specific needs and wants for external care providers, ensuring a holistic approach to support.

Our Holistic Approach

Meet Helen and Her Family

Helen is a 78-year-old woman living with Alzheimer’s disease. Her daughter, Sarah, has been her primary caregiver for the past five years. Recently, Sarah began to feel overwhelmed by the demands of caregiving while also managing her own family and career. She reached out to Guide2Care for support and signed up for the Essential Membership Plan.

Step 1: Initial Consultation and Understanding Needs

Our team began with an initial consultation to understand Helen and Sarah’s situation fully. We listened to Sarah’s concerns about her mother’s declining health and her own feelings of exhaustion and anxiety. We also spoke with Helen to understand her daily routine, medical needs, and personal preferences.

Step 2: Identifying Objectives for Assistance

Together, we identified the main objectives for assistance:

1. Ensuring Helen’s safety and well-being at home.
2. Providing Sarah with respite care to alleviate burnout and implementing practices like scheduling time for herself to prevent future exhaustion.
3. Exploring financial assistance options to support caregiving expenses.

Step 3: Outlining Specific Needs and Wants

Based on these objectives, we outlined the specific needs and wants:

– Daily home care services to assist Helen with activities of daily living (ADLs) such as bathing, dressing, and meal preparation.
– Regular visits from a nurse to manage Helen’s medical needs.
– Access to a local Alzheimer’s support group for both Helen and Sarah.
– Financial planning services to explore options for long-term care insurance and Medicaid.

Step 4: Implementing the Plan

With a clear plan in place, we connected Helen and Sarah with a reliable home care provider, arranged for regular nurse visits, and established communication with Helen’s primary doctor, who supported obtaining this assistance.

We also introduced them to a local support group where they could share experiences and receive emotional support.

We connected with financial planner who worked with Sarah to explore funding options, providing a sustainable financial plan for Helen’s ongoing care.

Step 5: Continuous Support and Monitoring

We maintained regular contact with Helen and Sarah, ensuring that their needs were being met and adjusting their plan as needed. This continuous support allowed Sarah to feel more at ease, knowing that her mother was receiving high-quality care and that she had a reliable support system. Now, she feels she can take time for herself without feeling she isn’t there for her mother.
