When Systems Don’t Connect, Families Carry the Weight
It can feel like every part of life is asking something from you at once — doctors, paperwork, benefits, family decisions, and everyday needs.
Care Navigation gives you a steady place to begin.
We slow things down, look at what is happening together, and help you take the next right step with more clarity and support.
What People Often Face
Trying to navigate the system
Arranging care at home
Only to have hours reduced or denied, without anyone explaining what to do next.
Dealing with insurance
Going back and forth, but not getting real help to move things forward.
Finding the right doctor
Without knowing where to start or who to trust.
Waiting on a medication approval
Going back and forth between your doctor and insurance, still without a clear answer.
Trying to make important decisions
Figuring out what kind of care is needed
Whether that’s assisted living, a nursing home, or memory care — and not knowing how to choose a place you can trust.
Thinking about life changes as we age
What matters most, and how to plan for it.
Putting things in place
Like a health care proxy or medical wishes — and not sure how to begin those conversations.
Wondering about hospice or comfort-focused care
What it really means — and when it becomes part of the conversation.
Over time, it can feel overwhelming, isolating, and exhausting.

Why I started this
Over the course of my career, I’ve seen how easily people can be left on their own during difficult health situations.
People were trying to get answers from doctors, waiting on responses from insurance companies, and moving between hospitals, specialists, and care providers — often without anyone helping them make sense of what was happening.
At the same time, they were trying to manage everything around it — finding stable housing, arranging transportation to appointments, figuring out how to afford care, or making sure basic needs were met at home.
Some were dealing with symptoms that didn’t have clear explanations. Others were trying to get care approved, understand medications, or know what step to take next.
None of these challenges exist on their own — they’re all connected.
What stayed with me most wasn’t just the medical side of it.
It was how often people felt dismissed, unheard, or left to figure things out on their own — not just in healthcare, but in everything surrounding it.
That experience led me to start Guide2Care — so people would have a place to turn, a way to be heard, and someone to help connect the pieces.
Over time, I realized that people don't need more information.
They need someone to listen, to understand, and to walk alongside them.
That’s what this work has become.
You don’t have to carry this alone.
Let’s talk about what’s happening, what feels disconnected, and what kind of support could help you move forward.
