You don’t need everything figured out to begin.
A focused conversation to understand your situation — whether you’re dealing with something immediate or beginning to think about what comes next.
We take the time to look at what’s happening across your care, clarify what feels unclear, and help you leave with a direction that feels manageable.
- Review of your situation before the call (if information is shared)
- 60-minute conversation focused on your concerns or questions
- Clarifying what’s happening across providers, insurance, or care needs
- Talking through planning ahead — including care preferences and decision-making
- Guidance on conversations around health care proxy or future care planning (if relevant)
- Identifying 2–3 clear next steps
- Written summary with recommendations after the call

We stay with you through changes, decisions, and follow-through — helping you keep things moving without carrying it alone.

When something needs attention right now.
Immediate, high-touch support during urgent situations — when decisions are time-sensitive and things feel overwhelming.
We help stabilize what’s happening, prioritize what matters first, and guide you through decisions in real time.

- Daily or frequent communication throughout the 7-day period
- Immediate help coordinating care, discharge, or placement decisions
